After you submit your ticket (the system will tell you what you need to fill in), you will get an answer (email notification) on how to proceed with the RMA.

I have just submitted a support ticket. But I've just realized that I forgot to fill my product number & serial number field. Would that be okay?

Or should I submit another ticket?

Did you mention the product name? If so, than you are fine. They'll ask for the S/N later on, normally.

The easiest way is to create a support ticket @ https://account.coolermaster.com and ask for a replacement part (part request). This is free of charge.
